Junior high and high school students (ages 13 through 12th grade) planning to attend MJC as a Dual Enrollment student must submit this form to the Dual Enrollment Team or Enrollment Services after completing their admissions application online. The form must be signed by a parent/guardian and the principal/counselor at your school.

This form gives Granted Petition to high school students wanting to register for Dual Enrollment sections listed on our Dual Enrollment Course flyer. Submit this form to the Dual Enrollment office to access registration for one of our current courses.
- MJC Activity and Student Rep Fee Waiver is now available online. You must be currently enrolled/registered in order to submit the waiver. Click the link, sign in using your single-sign on and complete the form. It will be submitted directly to Business Services.
